Are you a family member or family carer of someone who has a learning difficulty, a learning disability or autism?

 


This is your chance to have a real say in how health and social care in Cheshire West and Chester will work in the future.

 


Local disabled people and family carers are welcome to join in.

 


Health and social care are joining together to plan and run services. It is called Integrated Personal Commissioning.

 


Would you like to join the group which decides what should happen? It is called the Co-production group

 


People and their families will have more choice and control over their health and social care.

 


People with health and social care needs will only need one assessment.

 


Local people will take part in joint planning of services. This is called coproduction. It is more than just asking people what they think!

 


It is happening in Cheshire West, Vale Royal and Chester over the next 3 years.
